If you're stuck on the intake form and can't move forward or submit, try the following:
1. Check for unanswered required fields Scroll back through the form and look for any fields marked with a red asterisk (*) that may have been skipped. The form won't submit until all required fields are complete.
2. Refresh the page If the form appears frozen or the Submit button isn't responding, try refreshing your browser. Your progress may be saved β check whether your previous answers are still there before re-entering anything.
3. Try a different browser or device If the issue persists, try opening the form in a different browser (Chrome, Safari, Firefox) or on a different device. Some browser extensions can interfere with form behavior.
4. Clear your browser cache In your browser settings, clear your cache and cookies, then try again.
5. Still stuck? Contact us via the chat bubble at yupmd.com. Describe what step you're on and what's happening (e.g., "the Submit button does nothing" or "I get an error message on page 3") and our team will help you complete your intake.
